The lighting is one flash strobe with umbrella above the models. The skin is smoothed with the paint brush tool. Channel Mixer is almost always used in my images to desaturate the image a little, then adding a unifying color tone to it with Color Balance. You can define whether an image is warm or cool with Color Balancing.
